What we treat

Our home monitoring program can help if you have any of the following conditions:

How it Works

  1. Your provider will enroll you at your normal visit. You will receive a text inviting you to download the app at the appointment. If you didn’t get the text, give us a call so we can assist: 504.962.6202. You can also call this number if you’d like to self-enroll in one of our programs.

  2. Within two days of your visit, you’ll get a call from a member of our care management team. Our care managers will reach out to establish a care plan and mail you your monitoring device. The phone call will come from 866.899.3998.

  3. You’ll receive your monitoring device in the mail and can start recording your vitals. Your monitoring device will pair to your smartphone using the Unified Care App you’ve already installed. If you have any questions or run into trouble, call our support team at 866.899.3998.

  4. You’re all set! Remember to take your vitals as instructed. Our care team will review the results and reach out as needed.

What if I don’t have a provider?

If you'd like to self-enroll, that’s not a problem. We’ll schedule a brief video visit with one of our providers who will make sure you’re enrolled in the correct program.
