For Advanced Practice Students:

(School Affiliation Agreement with LCMC Health Required)
  • Step 1: Must have a current LCMC Health/School Affiliation Agreement
  • Step 2: School Faculty must submit Good Standing Attestation Prior to Clinical Rotation
    • Attestations must be fully completed and returned by the appropriate school official, NOT the student. Attestations returned by the student, or if incomplete, will not be accepted. The attestation form validates required items per Affiliation Agreement between School and LCMC.
    • Note: All students requesting access to EJGH’s Campus or Clinics will require health screenings, background checks and immunizations.
  • Step 3: Student's must review EJGH Orientation Experience Packet
  • Step 4: Student Submits APP Student Online Orientation Attestation

Our team will:

  • Locate the accepting physician
  • Connect sending and accepting physicians by a recorded physician-to-physician call
  • Confirm bed availability, obtain hospital approval and notify receiving departments
  • Assist in securing transportation
  • Obtain and document necessary patient information
  • Track transfer from time of call to patient arrival
  • Provide immediate feedback to close the loop with all referring physicians and facilities
