Need a copy of your medical records?

We offer several ways to request your medical records depending on the type of information you need and format.

MyHealthONE patient portal

MyHealthOne Portal consolidates many common tasks into one secure, easy-to-use online patient portal. It gives you access to most of your medical records on your desktop computer, laptop, tablet or smartphone 24 hours a day. MyHealthOne is currently not available for accounts after 12/31/2022. Fax or email

Please complete the following steps to obtain a paper copy of your medical records:

  1. Print and complete the Medical Records Release Form:

  2. Complete, sign and date the form. In order to verify your identification and validate your authorization, we require that you include a legible copy of a valid photo I.D. (e.g., driver's license, military I.D. or state I.D.)
  3. The documents can be sent by fax or email

  4. For assistance or to check on status please call 504.780.4520.
  5. To obtain Radiology Images, please contact the Radiology Department directly at 504-988-1747.
  6. To obtain a certified copy of your Birth Certificate please contact your state or local Vital Statistics Office.

Records delivered by mail will be shipped within 5-7 Business days after processing.

Records delivered by email will be received within 1-2 Business days after processing.

Effective July 1, 2023 - Cost for paper and electronic copies of your Medical Record

  • Record requested and delivered through electronic portals – No Charge
    • MyHealthOne
    • Health Record Wizard
  • Records send directly to another healthcare provider – No charge
  • Records delivered via email (outside of electronic portal emails), mail, fax, or in person to the patient - $6.50 plus tax
  • Records delivered to an insurance company, attorney, or other third party – Applicable fee will be charged to the requestor.

Urgent requests

For immediate continuity of care, your healthcare provider can request records.

The physician office must must email or fax 470-589-2665 a written request on their letterhead or fax coversheet indicating the patient’s name, date of birth and date of visit in the facility.

Insurance, attorney, disability and third party requests:
