Requesting your medical records

We offer several ways to request your medical records depending on the type of information you need and format.

MyHealthONE patient portal

MyHealthOne consolidates many common tasks into one secure, easy-to-use online patient portal. It gives you access to most of your medical records on your desktop computer, laptop, tablet or smartphone 24 hours a day. Some medical records may only be available through our hospital Medical Records office. MyHealthOne is currently not available for accounts after 12/31/2022. Fax or email

Please complete the following steps:

  1. Download, print and complete the authorization form. The authorization form must be signed and dated.

  2. In order to verify your identification and validate your authorization, we require a legible copy of a valid photo I.D. (e.g., driver’s license, military I.D. or state I.D.).
  3. You may send your request in the following ways:

  1. To obtain Radiology Images, please contact the Radiology Department directly at 504-988-1747.
  2. To obtain a certified copy of your Birth Certificate please contact your state or local Vital Statistics Office.

Records delivered by mail will be shipped within 5-7 Business days after processing.

Records delivered by email will be received within 1-2 Business days after processing

Effective July 1, 2023 - Cost for paper and electronic copies of your Medical Record

  • Record requested and delivered through electronic portals – No Charge
    • MyHealthOne
    • Health Record Wizard
  • Records send directly to another healthcare provider – No charge
  • Records delivered via email (outside of electronic portal emails), mail, fax, or in person to the patient - $6.50 plus tax
  • Records delivered to an insurance company, attorney, or other third party – Applicable fee will be charged to the requestor.

Urgent requests, records for your physician

For immediate continuity of care, your healthcare provider can request records.

The physician office must email lcmctulane@cioxhealth.comor fax 470-589-2665 a written request on their letterhead indicating the patient's name, date of birth, date of visit and the name of the facility where you were treated. Please indicate "STAT" for all urgent requests. For assistance, call 504.988.5261.

Non-patient requests (insurance, attorney, disability and third party requests)

Requests should be sent from your insurance company, attorney, or Disability Determination Service and be mailed to the address below. For assistance, call 504.988.5261.

LCMC Health recognizes the patient's right to access and obtain copies of their protected health information (PHI) in accordance to HIPAA laws. (Code of Federal Regulation 164) as well as Louisiana State Statutes.
