Senior Care
As we grow older, our health needs change. Our Senior Care team provides personalized support for adults age 65 and older, with a focus on helping each patient stay healthy, safe, and as independent as possible. At East Jefferson General Hospital, we care for seniors in both the inpatient and outpatient setting. We also work closely with family members and caregivers to make sure everyone understands the patient’s care plan.
Senior care during a hospital stay
Our inpatient Senior Care consult team includes a board-certified geriatric physician and nurse practitioner who care for hospitalized adults age 65 and older, particularly those with multiple medical conditions or complex care needs.
The team reviews the patient’s overall health, medications, ability to think and move safely, personal goals, and support needs after leaving the hospital. We work alongside pharmacists, social workers, hospitalists, case managers, and other members of the patient’s care team to help:
- Address health concerns related to aging
- Review medications and reduce the risk of side effects
- Identify changes in memory, thinking, mood, or behavior
- Support safe movement and help prevent falls
- Discuss the patient’s goals and care preferences
- Plan for a safe transition home or to another care setting
- Connect patients and caregivers with helpful services and resources
When appropriate, patients may continue their care at the Tulane Comprehensive Geriatric Clinic after leaving the hospital.
Senior care services
East Jefferson, in partnership with Tulane School of Medicine, offers the only Senior Care clinic in Jefferson Parish staffed by board-certified geriatricians. New patients as well as patients who need follow-up care after a hospital stay, are cared for by geriatricians who work with the patient’s primary care physician and other providers to address concerns related to aging and complex care.
Senior Care may be especially helpful for older adults who:
- Have several medical conditions or take multiple medications
- Have concerns about memory, confusion, mood, or behavior
- Have experienced falls or difficulty walking
- Need help maintaining independence and completing daily activities
- Have been hospitalized recently or experienced a change in health
- Need guidance about future health care wishes
- Would benefit from a care plan based on their personal goals and priorities
