While our name has changed, our commitment to delivering high-quality, compassionate care in the comfort of patients' homes remains the same.
LCMC Health Home Care provides Medicare-certified home health services for qualified patients who need skilled care at home. Care is provided on an intermittent basis under the direction of the patient's physician and may include nursing, therapy, education, recovery support, and help managing ongoing health needs.
Home health may be appropriate for patients who are recovering from surgery, managing a chronic condition, healing from an illness or injury, or need additional support after a hospital stay.
Referral information
Home health services require a physician's order and certain eligibility requirements must be met. Patients who may benefit from home health should talk with their physician about whether care at home is the right next step.
Hospital case managers, social workers, or nurses may also help begin the referral process once home health services are ordered. LCMC Health Home Care can assist patients and families with understanding eligibility requirements and verifying insurance coverage.
Available Services
Services are provided by a multidisciplinary team of healthcare professionals under the direction of the patient's physician and may include:
- Skilled Nursing
- Home Care Aide
- Physical therapy
- Occupational therapy
- Speech/Language pathology
- Medical social services
Specialized programs:

- Infusion therapy
- Wound care
- Congestive heart failure treatment
- Orthopedic recovery
- Neurologic rehabilitation
- Diabetes management
